IB refused my order, or I cancelled it, but the trade shows the position

What you see. The Commit dialog shows IB’s refusal message, or you cancelled a working order, and the trade still shows the position (or a closing order left the trade in Closed).

Why. By design today: Save & send writes the journal entry, marked filled at the prices on screen, before IB has filled anything. A later rejection or cancel does not remove it.

What to do.

  1. Check the trade. If the position is wrong, delete the action: Journal → expand the trade → ⋯ on the action → Delete this action. If it was a closing order, deleting the Close action reopens the trade.
  2. Only then send again, or the entry is recorded twice.

A send that fails because Tented is not connected writes nothing. See Save, send or both and Cancel an order.
