Pasting from thinkorswim fails or skips a fill

• “Unsupported or incomplete thinkorswim fill.” The copy was an order, or something other than a bought put, put vertical or balanced put butterfly (a call, a sale, a credit spread, a futures option). Copy a filled row, or use Record a combo.
• A paste skipped a fill as a duplicate. By design: duplicates are judged by time, contracts, quantities and combined cost, and identical fills at the same time cannot be told apart.
• Paste from thinkorswim is greyed out. All accounts is selected. Pick one account first (Add menu items are greyed out).
